Consent to Telehealth
Thank you for choosing SO SO THIN LLC and Beluga Health, P.A. (“Beluga”) for medical weight loss services via telehealth. All visits will be asynchronous (store-and-forward communications) unless a synchronous (real-time) telehealth visit is required by state law. Your practitioner is a board-certified physician licensed in your state.
Definition and Scope of Telehealth Services
Telehealth services provided through So So Thin LLC and Beluga Health, P.A. (“Beluga”) include consultations, assessments, and treatments delivered via electronic communications, such as secure messaging, audio-video conferencing, digital image transmission, health record review, and asynchronous store-and-forward interactions. Telehealth visits help enhance patient access to care, offer convenience, and can provide more efficient evaluations, particularly when in-person visits are impractical or unnecessary.

SO SO THIN LLC provides only administrative and operational support, facilitating connections between patients and licensed healthcare providers from Beluga Health, P.A. SO SO THIN LLC does not participate in clinical judgments, medical decision-making, or prescribing medications. All clinical services, including diagnosis, prescriptions, and treatments, are solely provided by licensed medical providers at Beluga Health, P.A.
Relationship with Primary Care Providers
Telehealth services offered through Beluga and supported administratively by So So Thin LLC are intended to complement—not replace—your relationship with your local primary care physician (PCP). Patients should maintain regular visits with their local PCP for comprehensive healthcare needs and regular examinations. Providers at Beluga offer specialty medical weight loss services and do not provide comprehensive primary care.
Possible Risks of Telemedicine
Potential risks associated with telehealth include, but are not limited to:
- Technology Issues: Interruptions or delays in medical evaluation or treatment due to technical failures or internet outages.
- Incomplete Information: Limitations resulting from the absence of a physical exam, potentially affecting the accuracy of diagnoses or treatment effectiveness.
- Security Concerns: Although rare, breaches of confidentiality or unauthorized disclosure of protected health information (PHI) may occur.
- Incomplete Medical History: Without complete medical records or patient disclosures, providers may unknowingly recommend treatments with unintended side effects, interactions, or complications.
- Emergency Care Limitations: Telemedicine services are not emergency medical services. If you experience symptoms that suggest a medical emergency, including but not limited to chest pain, difficulty breathing, severe bleeding, loss of consciousness, or stroke-like symptoms, immediately call 911 or visit the nearest emergency facility.
Expected Benefits of Telemedicine
Potential benefits of telemedicine include improved access to specialized medical care, increased convenience through elimination of travel, timely evaluation and treatment, and secure and efficient communication of medical information.
Service Limitations
While telehealth provides significant convenience, it also has inherent limitations. Because interactions occur remotely, healthcare providers cannot perform a hands-on physical examination. Consequently, certain medical conditions requiring detailed physical assessments may not be suitable for telehealth. Your provider will determine the appropriateness of telehealth for your specific medical needs. If your medical situation is deemed unsuitable for remote evaluation, you may be referred for an in-person examination with your local provider.
Your Rights and Acknowledgements:
- Privacy rights during telehealth are the same as an in-person visit.
- Personal medical information may be electronically communicated securely.
- All information provided becomes part of your medical record and is confidential.
- You may withdraw consent anytime by emailing info@belugahealth.com.
Additionally, by consenting to telehealth services, I acknowledge that:
- I understand that telehealth services offered by Beluga Health, P.A. do not replace my ongoing relationship with my primary care provider (PCP). I agree to continue receiving routine and preventive care from my local PCP.
- I understand that I may request to change to another Beluga healthcare provider at any time, and that I can request credentials, licensing details, and qualifications of my assigned provider.
- I acknowledge that all medical information obtained through telehealth interactions becomes part of my confidential medical record. Beluga Health, P.A. and So So Thin LLC take appropriate and necessary measures to protect the security, privacy, and integrity of my medical data in compliance with HIPAA and applicable state privacy laws.
- I am solely responsible for providing accurate, complete, and up-to-date medical history and relevant health information, including allergies, current medications, medical conditions, and previous diagnoses, to ensure proper care.
Informed Consent to Semaglutide and Tirzepatide
Semaglutide and Tirzepatide are medications used to assist weight management, which may be compounded formulations not yet FDA-approved. Your Beluga Health provider will evaluate if they are suitable for you based on medical history and conditions.
Risks include, but are not limited to, nausea, vomiting, gastrointestinal issues, hypoglycemia, potential thyroid tumors, pancreatitis, gallbladder problems, kidney issues, increased heart rate, and psychological effects.
Contraindications:
- Do not combine with other GLP-1 medications or weight-loss products.
- Avoid if pregnant, breastfeeding, or planning pregnancy.
- Not recommended with personal/family history of thyroid cancer or pancreatitis.
I acknowledge:
- NO GUARANTEE on treatment outcomes.
- OFF-LABEL USE for some medications.
- NO FDA APPROVAL for compounded medications used.
- NO REFUNDS: Medications are non-returnable.
- I must provide accurate medical history and updates to my provider.
- I must follow all instructions precisely.
Compounded Semaglutide
Note: Compounded medications prescribed by healthcare providers are not reviewed by the FDA for safety, effectiveness, or quality. FDA-approved semaglutide medications are available.
Important Safety Information
Immediately consult your healthcare provider or seek emergency medical attention if you experience any concerning side effects, including:
- Severe nausea or vomiting leading to dehydration: Maintain adequate fluid intake to avoid dehydration.
- Kidney issues or failure: Symptoms like vomiting, nausea, or diarrhea can lead to dehydration, potentially worsening kidney function in individuals with existing kidney conditions. Severe cases may require dialysis.
- Thyroid Tumors: Seek medical care if you notice difficulty swallowing, persistent hoarseness, lumps in your neck, or shortness of breath. (Refer to the Black Box Warning below.)
- Pancreatitis: Persistent, intense pain in your stomach or back.
- Gallbladder disease: Upper right or mid-stomach pain, fever, nausea/vomiting, or jaundice (yellowing of skin or eyes). Additional diagnostic tests may be required.
- Liver concerns: Symptoms like elevated liver enzymes, hepatitis, or jaundice.
- Diabetic retinopathy complications: Sudden vision changes, especially among individuals with type 2 diabetes and a history of diabetic eye problems.
- Severe gastrointestinal conditions: Not recommended if you have existing severe gastrointestinal diseases.
- Hypoglycemia (low blood sugar): While uncommon with GLP-1 medications, symptoms may include dizziness, shakiness, weakness, headache, rapid heartbeat, confusion, mood changes, sweating, anxiety, slurred speech, or lightheadedness. Consume about 15-20 grams of sugar (such as juice, regular soda, or candy) immediately and seek medical care. Consult your doctor if using with insulin or sulfonylurea medications.
- Elevated heart rate.
- Increased depression, mood alterations, or suicidal thoughts.
Never share compounded semaglutide with others.
Drug Interactions
Compounded semaglutide may interact negatively with alcohol and various prescription, non-prescription medications, or herbal supplements. Specifically:
- Increased hypoglycemia risk when combined with insulin or insulin-stimulating drugs (e.g., sulfonylureas).
- May delay gastric emptying, affecting absorption of oral medications.
- If taking thyroid hormone treatments, discuss timing adjustments with your prescribing doctor.
- Regularly monitor drug levels for medications with narrow therapeutic ranges (e.g., warfarin).
Refer to pharmaceutical literature provided for comprehensive side effects, interactions, and further safety details.

BLACK BOX WARNING:
GLP-1 medications are contraindicated for individuals with personal or family histories of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2. Animal studies have shown thyroid tumors and cancer with GLP-1 use; human risks remain unclear. Immediately inform your doctor if you notice symptoms such as trouble swallowing, persistent hoarseness, neck lumps, or breathing difficulty.
Additional Precautions:
- Inform your healthcare provider of your complete medical history prior to starting treatment.
- Avoid alcohol consumption.
- Discuss current diabetes medications with your provider before initiating treatment, as adjustments may be necessary.
- Use reliable contraception. Do not use semaglutide if pregnant, attempting pregnancy, or breastfeeding. Discontinue at least two months before planning pregnancy.
- Notify your healthcare provider about existing kidney, liver, or pancreatic conditions.
- Discontinue use and seek immediate medical attention if experiencing persistent severe abdominal pain, with or without vomiting, or signs of serious allergic reactions (swelling of face, tongue, lips, throat, difficulty breathing/swallowing, severe rash, fainting, dizziness, rapid heartbeat).
Severe allergic reactions, including anaphylaxis, have been reported. Discontinue immediately if suspected and seek urgent medical assistance.

HIPAA Privacy Statement
Beluga Health, P.A. and affiliates comply with HIPAA, using PHI for treatment, payment, healthcare operations, legal requirements, public safety, and specific regulatory activities.
Your Rights Under HIPAA:
- Inspect and copy PHI (fees may apply).
- Request amendments and receive an accounting of disclosures.
- Request restrictions on PHI use.
- Request confidential communications.
- Obtain a paper copy of this Notice.
- Be notified of any PHI breach.

- Minor’s Rights: If you are a minor (under the age of 18), you have the right to request that certain information related to certain sensitive services, such as reproductive health, mental health, or substance use disorder treatment, not be disclosed to your parent or guardian without your consent.
